SYTA Members are responsible for providing renewed documentation (eg, PI and PL Insurance Certificates or accredited First Aid Certificates
inclusive of HLTAID001, HLTAID002 and HLTAID003 valid for 3 years) to SYTA at time of expiry to maintain their Full Membership. If your
Insurance or First Aid has been updated since July 1 last year and evidence of your certificates have not been provided
to SYTA, you will need to upload copies with this application.
SYTA renewing members are responsible for providing a completed Professional Development Activity form showing 12 points earned in the prior
membership year (July to June) as per SYTA's PD criteria. Please upload your completed activity form with this application. Blank forms
